Regenerations need the exhaust to stay hot for long enough to burn the soot away. Short journeys, a faulty sensor, EGR or injector faults and oil in the exhaust all interrupt them, and the filter fills up.

A new filter is often over £1,000 fitted, depending on the car. Ash and cracks can't be cleaned, and the readings will show if either is the problem.
